Parent Volunteers Needed - Please print, complete and return to Mr. Fox or Mrs. Arnold

Media Release- ALL STUDENTS must have this signed by a parent to attend Kick-off, Detroit Regional or West Michigan Regional.  Please complete and submit to Mrs. Arnold

Liability Waiver - ALL STUDENTS must have this signed to go to any events.

Health and Medical Release - ALL STUDENTS must have this signed to go to any events that require travel over 40 miles and/or overnight travel.

Student Travel by Auto - Must be signed by parents for students riding in a vehicle other than their own parent or guardian.

Private Transportation Driver Guidelines- Any parents who volunteer to take other students to events must print, complete and give to Mrs. Arnold.
